What Is a Slipped Disc and Why It Does Not Always Need Surgery

Let me clear up the biggest misconception first. Your disc does not actually slip out of its place. That is not how it works. The term "slipped disc" is misleading, and unfortunately it scares a lot of patients into thinking something has moved out of position and needs to be pushed back surgically.

Here is what actually happens. Your spine has soft cushions between each bone, called intervertebral discs. Each disc has a tough outer ring (called the annulus) and a soft, jelly-like center (called the nucleus). When the outer ring weakens or tears, the soft center pushes outward. That is what people call a slipped disc.

Now, depending on how far that soft center has pushed out, it is either a disc bulge or a disc herniation. And this difference matters a lot.

Disc Bulge vs Disc Herniation: The Key Difference

A disc bulge is when the disc swells outward slightly but the outer ring is still intact. Think of it like pressing your thumb into a balloon. The balloon changes shape but does not pop. This is extremely common and in many cases, it does not even cause pain. Studies have shown that 30 to 40 percent of people with absolutely no back pain have disc bulges on their MRI. So a disc bulge on an MRI report does not automatically mean that you need surgery immediately.

A disc herniation is a little different. This is when the outer ring actually tears and the soft inner material leaks out. When this leaked material presses on a nearby nerve, that is when you get severe pain, numbness, or weakness in your leg. This is the situation that needs treatment. But even here, the treatment does not have to be surgery.

The important thing to understand is this: your MRI report alone should never decide your treatment. What matters is how you feel, what your physical examination shows, and whether the MRI findings actually match your symptoms. I have seen hundreds of patients whose MRI showed a scary looking disc herniation, but their symptoms were mild and responded beautifully to non-surgical treatment.

Which Disc Levels Cause the Most Pain: L4-L5 and L5-S1

Your lower back has five vertebrae, labeled L1 through L5. Below L5 sits the sacrum (S1). The two disc levels that cause the most problems are:

L4-L5: This is the most commonly affected disc in the lower back. When this disc bulges or herniates, it typically causes pain in the outer thigh, the front of the calf, and sometimes the top of the foot. Many patients describe it as a burning or aching sensation that starts in the lower back and travels down the leg.

L5-S1: This is the second most common problem area. Issues at this level tend to cause pain that shoots down the back of the thigh, into the calf, and down to the heel or sole of the foot. This is the classic sciatica pattern, and it is often described as a sharp, electric shock type of pain.

When a patient tells me that the pain starts in their lower back and travels all the way down to their foot, in most cases I already have a good idea which disc level is the problem before even looking at the MRI.

When Do You Actually Need Surgery?

I am not saying surgery is never needed. Surgery absolutely has its place. But it should be the last resort, not the first suggestion. In my clinical experience over the last 10 years, I can say confidently that surgery is truly necessary in only about 10 to 15 percent of slipped disc cases.

Surgery is needed when:

  • The patient has developed significant weakness in the foot or leg (a condition called foot drop)
  • There is loss of bladder or bowel control (this is a medical emergency)
  • The patient has tried proper non-surgical treatment for months with no improvement at all

Non-Surgical Treatments That Actually Work

Now let me tell you about the treatments that I use in my practice every day and that have helped thousands of patients avoid unnecessary surgery.

Medicines and Exercises

Most patients who have back pain or leg pain do well on a short course of medicines and then exercises. In fact 70 to 80 percent of my patients get better with this only. Of course they have to take precautions which I tell them to avoid putting a lot of strain on their spine.

Epidural Steroid Injection

This is one of the most effective and well-studied treatments for slipped disc pain. Here is how it works.

Using fluoroscopy (which is basically a live X-ray) or ultrasound guidance, I place a thin needle right next to the area where the disc is pressing on the nerve. Through that needle, I inject a combination of a powerful anti-inflammatory steroid and a local anaesthetic. The medicine reduces the swelling around the nerve, and once the swelling goes down, the pain reduces dramatically.

The entire procedure takes about 30 to 40 minutes. You do not need to be admitted. There is no cut, no stitch, no scar. You walk in, get the injection, rest for about 30 minutes, and go home the same day.

Most patients get good improvement over 3 to 4 weeks which can last for months or even years. But it all depends upon many other factors like a patient's weight, activity level and age.

Ozone Discectomy: How Ozone Gas Shrinks the Disc

This is a treatment that many patients have not heard about, but it has been used successfully in Europe and India for over a decade.

The concept is simple. I use a thin needle (again under fluoroscopy guidance) to inject a precise amount of medical-grade ozone gas directly into the damaged disc. The ozone reacts with the disc material and causes it to shrink. When the disc shrinks, it stops pressing on the nerve, and the pain goes away.

Think of it this way. Imagine a balloon pressing against a wire. Instead of cutting the wire or moving it surgically, we just let a little air out of the balloon. The pressure releases naturally.

Ozone discectomy takes about 30 minutes, is done under local anaesthesia (you are awake the entire time), and requires no overnight stay. Most patients can return to their normal routine after 4 to 5 days.

Transforaminal Nerve Root Block

When I can pinpoint exactly which nerve root is being compressed (for example, the L5 nerve root or the S1 nerve root), I use a very targeted injection called a transforaminal nerve root block.

This is even more precise than a regular epidural injection because the medicine is delivered right to the specific nerve that is causing the problem. Everything is done under fluoroscopy guidance, so accuracy is not a concern.

Patients typically feel significant pain relief within 48 to 72 hours. This treatment works especially well for patients with sciatica, where the pain clearly follows the path of one specific nerve.

Sciatica is not a disease by itself. It is a symptom. It means that the sciatic nerve, which is the longest and thickest nerve in your body, is being compressed or irritated. And the most common reason for that compression is a slipped disc at the L4-L5 or L5-S1 level.

If you have sciatica, you will recognise these symptoms immediately:

  • A sharp, shooting pain that starts in the lower back or buttock and travels down the back of your thigh, into your calf, and sometimes all the way to your foot

Week 1 to 2: This is when most patients see a significant drop in pain levels. Daily activities like walking, sitting, and sleeping become much more comfortable. Many patients who could barely walk before treatment are moving around freely by the end of the second week.

Week 3 to 4: By this point decent improvement is typical. This is also when I start recommending gentle back strengthening exercises and posture correction. The treatment has done its job of reducing pain and inflammation. Now it is your turn to build strength so the problem does not come back.

Month 2 to 3: Full recovery and rehabilitation phase. The focus shifts from pain management to long-term prevention. This includes core strengthening exercises, proper sitting and lifting techniques, and maintaining a healthy weight.

One thing I always tell my patients: the injection removes the pain, but you have to build the strength. Treatment without rehabilitation is like fixing a flat tyre but continuing to drive on a bad road. Both parts matter.
